Garbage including trash bags, a soda can, animal droppings and metal pieces were observed within the operation's playground. Note: This was corrected at inspection when provider removed the garbage items from the playground.

A food item was served to a child with a known food allergy during lunch.

746.2605(1)(A) - Furnishings and Equipment for Pre-Kindergarten Age Interest Centers Clearly Defined

There are no well defined interest centers in the prek and school-age rooms.

746.5603(c) - Types of Vehicles Used to Transport Children - Be Maintained Safely

Two of the seats in the transportation vehicle are missing plastic covers exposing metal components.

746.2203(b) - Written Activity Plans - Plan Followed by Caregiver

Caregivers were not observed following the lesson plan posted and planned for the day/week.

746.3701 - Safety - Areas Free From Hazards

Hazards such as staples and push pink tacks were observed within the toddler and prek rooms and accessible to children. The toddler room has tiles that have lifted presenting a tripping hazard. The outdoor fence has protruding nails and there are screws and utility items exposed and accessible to children. Glass door that leads to the infant room its not marked at the children's eye level.

746.201(2) - Written Personnel Policies

Personnel Policies, Operational Policies, Orientation Curriculum and Pre-Service Curriculum were not available for review at the time of inspection.

746.1301(a)(2)(A) - Training Requirements for Employees, Caregivers, and Directors - Pre-Service Training

A caregiver with no prior experience and employed for over 90 days has not completed pre-service training.

746.3407 - Maintenance of Building, Grounds and Equipment

Infant room: walls were observed with chipping paint, a ceiling tiles is out of place/curved and one of the cribs has milk residue. Toddler / Prek and School-Age rooms were observed with accumulated dust / dirt on the floors, walls/furniture, ceiling fans and parts of the side molding are out of place exposing the wall material.

746.3317(9) - Food Service and Preparation Requirement - Food Prep Area Separate from Other Areas

A sheet of baked brownies was uncovered and stored underneath the kitchen cabinet along with cleaning products. Note: This was corrected at inspection when provider covered and stored the brownies at a different location.

746.5202(1) - Emergency Preparedness Plan - Evacuation, relocation, and sheltering/lock-down procedures

Emergency Prepardness Plan was reviewd and is missing the following information: (1)(A) The first responsibility of staff in an emergency evacuation or relocation is to move the children to a designated safe area or alternate shelter known to all employees, caregivers, parents, and volunteers; (1)(B) How children will be evacuated or relocated to the designated safe area or alternate shelter, including specific procedures for evacuating and relocating children who are under 24 months of age, who have limited mobility, or who otherwise may need assistance in an emergency, such as children who have mental, visual, or hearing impairments; (1)(C) The staff responsibility in a sheltering/lock-down emergency for the orderly movement of children to a designated location within the center where children should gather; (1)(D) An emergency evacuation and relocation diagram (1)(E) Name and address of the alternate shelter away from the center you will use as needed; and (1)(F) How children in attendance at the time of the emergency will be accounted for at the designated safe area or alternate shelter. (2) Communication, including: (2)(A) The emergency telephone number that is on file with us; and (2)(B) How you will communicate with local authorities; and (3) How your staff will evacuate and relocate with the essential documentation including: (3)(A) Parent and emergency contact telephone numbers for each child in care; (3)(B) Authorization for emergency care for each child in care; and (3)(C) The child tracking system information for children in care; (4) How your staff will continue to care for the children until each child has been released; and (5) How you will reunify the children with their parents as the evacuation, relocation, or sheltering/lock-down is lifted.
