18-23 month classroom were observed eating gold fish on the table surface. This was corrected at inspection.

At the time of inspection the infant room was out of ratio with 5 to 1 caregiver, and the 2's room was out of ratio with 13 to 1. This was corrected at inspection by transitioning children.

CPSC form was not updated annually with a date of 2/5/2025. This was corrected at inspection.

745.641 - AP Background check results - Must receive notification prior to allowing subject to be present at your operation

At the time of inspection, 2's caregiver did not have an active background check and had direct access to the children. The caregiver left the operation. This was corrected at inspection.

746.2805(8) - Prohibited Punishments - Restrictive device for time out

Based on the information obtained during the investigation, it was found that the caregivers utilized the highchair not only for feeding purposes, but also as a method to manage the child's behavior.

746.5101(b) - Annual Fire Inspection - Conducted by Fire Marshal

During the monitoring inspection, the fire inspection expired on 12/3/2024.

746.2207(a) - Screen Time Activities - Not Used for Under Two

During the monitoring visit, the television was on in the classroom. There were four children present, and two of the children were 9 months and 10 months old. The two other children were 2 years old. The caregiver turned the television off upon request from the inspector.

During the follow up visit, there were two caregivers with pending background checks. The caregivers were not able to leave the operation due to being out of ratio. The caregivers were located in the infant and toddler classroom.

During the initial visit, the infant classroom had one infant, age 20 mos., watching screen time on an iPad in the classroom. The caregiver removed the device upon request from the inspector.

During the monitoring visit, there was one infant who did not have an up-to-date feeding form available for review. The last form completed was in June.

During the follow-up visit, there were two children without up-to-date shot records.

During the monitoring visit, the CPR was completed online.

During the visit, two caregivers did not have CPR/first aid certificates. One caregiver certificate for CPR/first aid has expired. The second caregiver did not have the CPR/first aid certificate available for review.

During the visit, the infant classroom had three cribs not labeled with the infants's names. On one crib, the infant's name on the outside was incorrect.

This standard was evaluated with regard to an investigation and a violation was found. The operation did not provide the feedings to a young child at the schedule time request in between feedings from the written feeding form signed by the parent.

During the monitoring visit, the infant classroom had six children in the classroom with one caregiver. The specific age is 10 months.

During the monitoring visit, the operation was not able to provide any documentation for fire, lockdown, or sheltering drills.

During the monitoring visit, the operation did not have the monthly daily ground checklist available for review.

746.3407 - Maintenance of Building, Grounds and Equipment

During the monitoring visit, the grass is overgrown on the playground, and weeds are growing inside of the playground structure under the tire mulch.

746.3407(12) - Mark Cleaning Supplies, Toxic Materials; Keep Separate from Food and Inaccessible to Children

During the monitoring visit, the operation had four cleaning supply bottles that were not labeled in the pre-K classroom, infant classroom, and toddler classroom.

746.2507(7) - Activities for Toddlers - Provide Social and Emotional Development

During the monitor visit, the toddler classroom has shelves but limited toys. The classroom does not have toys available for social-emotional, fine motor, or art activities or manipulatives, and the bookshelf is turned toward the wall.

746.2407 - Infant Care Area Furnishings and Equipment -Use of Safety Straps

During the monitoring visit, there was an infant in the high chair that the caregiver was feeding. The safety straps were not on the infant in the highchair. Upon request, the caregiver added the straps to the infants.

746.2417(2)(B) - Activities for Infants - Not in Restrictive Device for more than 15 Minutes

During the monitoring visit, there was an infant in the restrictive device for twenty-five minutes. The caregiver removed the infant upon the director's request.

During the monitoring visit, the operation was not able to provide any documentation for carbon monoxide detection.
