The admission information form was missing the following information: Address, and telephone number of another responsible individual (friend or relative)who should be contacted in an emergency when the parent cannot be reached, Names and telephone numbers of persons other than a parent to whom the child may be released, Authorization to obtain emergency medical care and to transport the child for emergency medical treatment, A statement of the child's special care needs.

745.641 - AP Background check results - Must receive notification prior to allowing subject to be present at your operation

The primary caregiver allowed a person to be present at the operation before receiving notice from the CBCU regarding the person's eligibility. This was corrected during the inspection when person left the LCCH after being communicated about the rule.
