Cots used in a classroom were not assigned to children. This was corrected at inspection when the caregiver assigned cots to children by numbering cots and assigning to each child.

One caregiver stepped out of classroom that required two caregivers to maintain ratio. This was corrected at inspection when the caregiver was notified and she went back to classroom.

4 of 8 children were missing a statement on special care needs.

1 child did not have a signature from a health care professional after being enrolled for more than 12 months.

All booster seats in a van they used to transport children the day before were expired. This was corrected at inspection when a staff member went to store and bought new ones.
