The crib was observed with 2 blankets and a baby mobile in it. The child was not inside the crib. The child was later laid in the crib with the items left inside.

A caregiver was observed not washing hands after diapering a child. The caregiver used gloves which are for single use

The center must post in a prominent place, the emergency evacuation plan, current menus, a list of current employees, keeping children safe posting and emergency contact numbers.

A child was observed with a sippy cup of milk inside the crib at the end of naptime. this was corrected when the caregiver removed the cup.

A caregiver was observed not having hands on a child on an above floor level diapering table while changing the child.

The walls in toddler, infant, preschool and bathrooms were observed, and remain uncleaned.

The caregiver and children did not wash hands after coming inside and prior to eating lunch.

Two children were observed going to the bathroom to wash hands unsupervised.

The sink, walls and floors in two rooms and in the bathroom had not been wiped clean. A trash can in one of was observed without a lid, and the can was dirty and accessible to the children.

4 children were observed strapped in restrictive devices to keep them from leaving the classroom and wandering into other areas.

746.3407(9) - Floors, ceilings and Walls in Good Repair and Clean

The walls in each room was observed and were dirty. The boy's bathroom toilets and urinals had not been cleaned or flushed. There were smells of old urine in the bathroom. Spots on the walls in three of the rooms had peeling paint.

Personnel and training records had not been maintained. The files were incomplete for signatures and submission.

Electrical cords coming from an electrical surge protector was exposed and accessible to the children. This was corrected when the caregiver removed the surge protector and covered the wall plug with a cover.

The operation did not have a copy of the annual health inspection for review.
