The operation did not have working sinks in the diaper changing areas in multiple rooms.

746.201(9) - Complying with Laws and Rules

The permit holder did not complete the tasks specified by the Health Department, that were due 3/12/26 by 3:30 PM. This included cleaning, sanitizing, and having working sinks.

746.3407 - Maintenance of Building, Grounds and Equipment

The operation was not clean and maintained. There were mouse feces and urine in all rooms of the building, in and on shelves, cubbies, the floor, and around food.

746.305(a)(4) - Report Outbreak of Communicable Illness

A child was diagnosed with a communicable disease and licensing was not notified.

746.307(d) - Parental Notification of Communicable Disease

A child in care was diagnosed with a communicable disease and parents were not notified.

The infants were not getting hands washed after diapering.

Staff set sticky traps and bate traps with baking soda and cornmeal throughout the daycare and are not certified in pest control.

It was found during the investigation that staff are using their cell phone for personal use while supervising children.

746.3407 - Maintenance of Building, Grounds and Equipment

There were mice droppings and urine on the shelving, along walls, in the kitchen, storage areas, and the classrooms. Additionally, there were mice droppings found in the toy cubbies that children play with, and in cubbies where children's personal belongings are kept.

746.1201(1) - Responsibilities of Employees and Caregivers -Demonstrate Competency, Good Judgment, Self-control

Staff did not demonstrate good judgement when they did not replace the lids on the snack totes in the kitchen, and a mouse was found in one of the totes, and the mouse had eaten into the snacks in the tote.

746.5625(a) - Electronic Child Safety Alarm - is equipped in each vehicle

the vehicle that was purchased does not have the safety alarm installed in it.

746.1609 - Maximum Group Size 13 or More Children

The class had exceeded the goups size. There was 12 children and the maximum group size is 10.

746.1605(1) - Child/Caregiver Ratio - 18 Months Difference in Age

There was a 17 month old infant in the class with a 5 year old, which exceeds the 18 month gap.

746.1601 - Child/ Caregiver Ratio - 13 or More Children

The infant class was out of ratio by one and the preschool class was over by 2 children.
