Two children were missing health statements. Three children were missing the emergency contact address. Two children were missing parent signatures on the child-care enrollment agreement.

A caregiver failed to supervise children when children ages 3 and 4 years old were left alone in the restroom and the classroom. The director failed to supervise a child when the child was left alone in the office.

One caregiver did not have a background check submitted. This was corrected when the caregiver left the operation.

Two staff did not have personnel files available for review. One staff was missing a pre-employment affidavit. Four staff were missing proof of orientation. Five staff were missing high school diplomas. One staff did not complete 24 hours of annual training in the past year.

A volunteer was present and had access to children without the operation submitting a background check.

The infant classroom had five infants with one caregiver. The specified age group of the classroom is 0-11 months old which requires a child/caregiver ratio of 1 caregiver to 4 infants. This was corrected when the director stayed in the classroom until another caregiver returned from their lunch break.

746.2426(a)(1) - Infant May Not Sleep in a Restrictive Device

One infant was sleeping in a stroller. This was corrected when the infant was moved to a crib.

746.2428 - No Swaddling of Infant Unless Completed Sleep Exception Form on File

One infant was sleeping in a swaddle with their arms restricted. This was corrected when the swaddle was taken off the infant.

746.5101(a) - Annual Fire Inspection - Before Initial Permit Issued and Every 12 Months

The operation did not have a current fire inspection.

One infant was in a stroller without the safety straps fastened. This was corrected when the infant was removed from the stroller.

A toddler was observed sleeping with a bottle. This was corrected when the bottle was removed.

One infant was sleeping with a bib on and a pacifier clip attached to the bib. This was corrected when the bib and pacifier clip were removed.

The operation did not have a current sanitation inspection.

746.4135(b) - Children's Products- Annual Certification

The operation did not certify a annually in writing using a form provided by DFPS that you have reviewed each of the recall notices issued by the CPSC and that there are no unsafe products in the center.

746.2415(b) - AP Cribs - Only Snug Fitting Sheet and Crib Mattress Cover Allowed if Infant Younger than 12 Months of Age

One infant was sleeping in a crib with a blanket in the crib. This was corrected when the blanket was removed from the crib.

746.4907(d) - Installation and Maintenance of Loose-fill Surfacing - Proper Depth Maintained

The depth of the loose fill under a slide exit is not maintained at six inches.
