746.611(a)(3) - Health Statement parent sign - Dated within past yr, name/address health care professional, states child can participate. Follow by signed statement

Two of the four children's files reviewed did not have the required health statement on file and it has been more than 12 months since they enrolled.

746.1701 - Child/Caregiver Ratio - 12 or Fewer Children

The operation was out of ratio while one caregiver went to cook lunch leaving one caregiver with 2 children 0-17 months and 6 children 18 months to 3 years old.

746.1205(a)(4) - Supervision ensures each child?s safety, well-being, including physical proximity and auditory or visual awareness of each child?s on going activity

There were three instances when the caregiver left the classroom with some or all of the children remaining in the classroom alone. This prevented the caregiver from having the needed physical proximity to the children if a situation were to arise and they needed to intervene.

746.3309(c) - Parent Provided Meals/Snacks - Provide Safe Storage and Service

A frozen meal was left out on a counter top for at least an hour and twenty five minutes before it was cooked and served to a child in care. The box states to keep frozen and do not thaw. Additionally, one child was served pizza that had been sitting out on the counter for the same amount of time.

746.3407 - Maintenance of Building, Grounds and Equipment

There was a bag of trash that was accessible to children outside on the playground as well as multiple protruding nails from the fence and a broken fence picket. Also outside was a rake that was accessible to children and there were multiple tall weeds/grass. Inside the operation, multiple table tops had not been cleaned as they had food or bubble residue on them and there were multiple spots throughout the operation that the floor needed to be swept or mopped due to dirt/debris and food.

746.3415(7) - Employee Handwashing After Handling or Cleaning Bodily Fluids

A caregiver wiped the noses of three children without washing thier hands.

746.3301(f)(1) - Nutrition and Food Service - Drinking water available at every snack, mealtime, and during and after active play

Children did not initially have water outside and had to wait for it to be brought out. Additionally, only one child had water available to them during snack..

746.4135(b) - Children's Products- Annual Certification

The posted CPSC form expired 5/2/2025.

746.3407 - Maintenance of Building, Grounds and Equipment

Four sinks at the operation that are used by children had debris on them and needed to be cleaned. Additionally, an open bag of potting soil was on the floor by the backdoor and had leaked out onto the floor.

746.1205(a)(2) - Responsibilities of Caregivers - Know number of children responsible for

Two caregivers were asked at different occasions how many children they were responsible for and both caregivers stated they had 9 children that they were responsible for/in care. The children were counted with one caregiver and there was 10 children present during the inspection that the caregivers were responsible for.

746.2417(2)(B) - Activities for Infants - Not in Restrictive Device for more than 15 Minutes

An infant was observed seated in a restrictive device for 20 minutes.

746.3401(a) - Annual Sanitation Inspection

The operation did not have a current sanitation inspection on file for review during the inspection.
