The diaper changing mat in the toddler room was torn with the interior padding exposed. Note: The diaper changing mat was replaced with a new one during the inspection.

Based on the evidence gathered during the investigation, a caregiver did not provide the level of supervision necessary to ensure a child's safety as caregiver was not aware of when child inserted crayon into ear until child expressed discomfort.

746.305(a)(2) - AP Report Child Injury Requiring Medical Treatment by a Health-Care Professional or Hospitalization

Based on the evidence gathered during the investigation, the operation did not contact licensing within two days of a child requiring medical treatment from a medical professional.

Operational policies was missing the safe sleep policy.
