746.3503(a) - Diaper Changing Equipment - Smooth, Non-absorbent, Easy to Clean Surface

The changing mats on the changing tables in two classrooms have a few tears and rips in them there have been taped over.

746.1601 - Child/ Caregiver Ratio - 13 or More Children

A caregiver providing care to seven children, ages seven months to thirteen months was over ratio by two children. Note: This was corrected at inspection when a second caregiver was placed in classroom.

746.2207(a) - Screen Time Activities - Not Used for Under Two

During the course of the investigation, it was determined that a caregiver used screen time for children under two years of age in the infant classroom.

It was observed of a child down the hallway standing out side a classroom door with out a caregiver present.

746.611(a)(1) - Required Health Statement - Written, dated within past year, indicates child is physically able to participate in operation's child-care program

One out of nine children files did not have a health statement to review.

746.501(a)(18) - Written Operational Policies - Promotion of Physical Activity

Promotion of indoor and outdoor physical activity was not available for review in the operational policy.

746.3701 - Safety - Areas Free From Hazards

Part of the playground is being used to store extra indoor and outdoor equipment. Storage areas should be inaccessible to children.

746.3407(13) - Using, Storing and Disposing of Hazardous Materials

A storage closet containing cleaning supplies was unlocked. The closet is located in a children's bathroom stall and is accessible to children when not locked. This was corrected at inspection when the lock was realigned and the door was secured.

746.2419(8) - Requirements for Feeding Infants - Other Use of Handwashing Sink

Milk jugs were observed being stored in the hand washing sink in two rooms. This was corrected at inspection when a discussion was had with the caregivers about the use of hand washing sinks and the jugs were removed.

746.2421(b) - Written Feeding Instructions - Review and Update Every 30 Days

None of the five infants who do not eat table food had infant feeding instructions on file.

746.3505(b) - Diapering Procedures - Wash Infant's Hands

An infant was observed being changed and not having their hands washed. This was corrected at inspection when a discussion was had with the caregiver and she washed the child's hands.

746.405(a) - Required posting of Emergency Telephone numbers

Emergency numbers were not posted.

746.4751(c) - Maintenance Procedures for Active Play - Hazards Removed or Repaired

A bench is separating and has exposed screws.

746.203(2) - Liability Insurance Requirements - Provide Proof of Coverage Annually by Anniversary Date of Issuance

Proof of insurance for the new ownership was not available.
