746.1203(3)(A) - Administrative and clerical duties that take caregiver's attention away from children

The operation had one operational staff for the entire building and was not able to assisted with the inspector to perform his licensing duties during their annual inspection to evaluated the minimum standards.

746.3401(a) - Annual Sanitation Inspection

The operation has not complete their annual sanitation inspection since 11/15/2024.

746.3001(2) - Field Trips - Emergency Medical Forms

The operational staff did not carry the list of emergency medical consent forms and emergency contact information for each child that attend the field trips for date of March 3, March 4, March 5 and March 6.

The operation did not have written list of all children that went on the field trip for licensing to review for the date March 3, March 4, March 5 and March 6.

746.4403(a) - Handwashing Sink in Diaper-Changing Area

A total of eight classroom were observed. Three out eight classrooms did not have one hand-washing sink in each diaper-changing area.

746.3503(a) - Diaper Changing Equipment - Smooth, Non-absorbent, Easy to Clean Surface

A total of eight classroom were observed. Two out of eight classroom did not had a changing diaper changing table and another classroom did not have a station bed for children use.

746.2409(a)(3) - Safety Requirements for Cribs - Waterproof, Washable Mattress

The operation has a total of one infant classroom. Four out of six infant cribs do not have the infant mattress for licensing to review.

746.2605(1)(B) - Furnishings and Equipment for Pre-Kindergarten Age - Interest Centers Used Independently

A total of eight classroom were observed. Four out of eight classroom needs a storage bin, toys accessories and equipment and other children desk and tables.

746.3407 - Maintenance of Building, Grounds and Equipment

A total of eight classroom and one playground were observed. All eight classroom were not properly clean and sanitized. Trash and other items were observed inside the playground.

746.2409(a)(9) - Safety Requirements for Cribs - Documentation that Crib Meets Applicable Federal Rules

The operation has a total of one infant classroom. The operation did not have the documentation for the infant crib equipment as required 746.2409(9).
