A caregiver without notification from CBCU stating they have an eligible background check was present and had contact with children in care.

Fire drills are required for several purposes. The children learn an important procedure to help ensure their safety should an evacuation be made.

Several employee files were missing the 2985, notorized Affidavit in their personal files.

746.603(a)(3) - Children's Records - Health Statement

Some children's files were missing the Health statement from the child's physician or an equal. This statement must be placed in each file to show that the child is able to participate in a child care setting.

746.5607(a) - Child Passenger Safety Seat System - Must Use for Each Child in When Transporting and be Properly Installed

During a DFPS investigation, it was determined that a child in care who was being transported was not buckled properly. As a result, the child fell forward and hit their head.

During a DFPS investigation, it was determined that a caregiver who was conducting transportation, was in a hurry and did not ensure that all children were buckled in properly. As a result, a child in care fell forward hitting their head.

746.1201(4) - AP Responsibilities of Employees and Caregivers -Ensure No Child Abused, Neglected, or Exploited

This standard was found deficient as part of a DFPS Investigation.

746.3605(b)(1) - Child Becomes Ill in Care and Requires Immediate Treatment or Hospitalization - Contact EMS or take to ER

A child in care became unresponsive and required CPR to be performed by the operation's staff. The operation did not contact emergency medical services or take the child to the nearest emergency room.

746.305(a)(2) - AP Report Child Injury Requiring Medical Treatment by a Health-Care Professional or Hospitalization

A child in care received CPR and was later transported to the hospital by their parent. The child received medical treatment, but the operation did not notify CCR.
