A caregiver did not know the number of children they were responsible for,

746.801(3) - Required Records Maintained and Made Available - Personnel and Training Records

The director's training records were not available for review,

746.2426(a)(2) - Infant May Not Sleep in a Restrictive Device - Remove the infant from the device and place the infant in a crib as soon as possible

One infant, 4 months of age was observed napping in a restrictive device for approximately 5 minutes. Licensing staff walked into the infant classroom at 10:17AM and observed one infant sleeping in a restrictive device. Licensing departed the classroom at 10:24AM, in which the infant was still in the restrictive device.

746.1201(1) - Responsibilities of Employees and Caregivers -Demonstrate Competency, Good Judgment, Self-control

One child two years of age was placed in a highchair during circle time activities, diaper changing, naptime services, and/or while engaged in meals/snacks. Based on the information obtained, the child would be placed in a highchair for a 10?20-minute period, including while staff was conducting diaper changing task and/or during circle time. The caregiver's/staff did not demonstrate competency, and/or utilize good judgment by utilizing a highchair as form of keeping the child in control.

746.5101(a) - Annual Fire Inspection - Before Initial Permit Issued and Every 12 Months

The operation did not have a fire inspection conducted for Licensing to observe.

746.3407 - Maintenance of Building, Grounds and Equipment

Nap mats observed in the Toddler classroom were torn apart to the extent of exposing the foam surfacing that children are placed on. The nap mats cannot be properly sanitized due to the exposure of surfacing material.

746.2419(6) - Requirements for Feeding Infants - Label Bottles/Cups

Two bottles in the infant classroom were not labeled with the infant's first, and/or last name.

746.5015(c) - Safety Requirements for Sprinkler Play - Equipment and Water Hoses Kept Out of Child's Reach

A water hose was observed on the outdoor area unraveled, and accessible to children in care.
