The two bus binders were missing the parent's names, telephone numbers and emergency telephone numbers for each child being transported.

There were broken toys outside on the playground. This was corrected at inspection when they were removed from the playground and out of the reach of the children.

In the toddler classroom, there was shaving cream and clorox wipes in the reach of the children. The items were moved out of the reach of children.

The outside playground has mulch that does not meet the proper 6 inches required. The mulch measures 2 to 3 inches.

The operation failed to update the employees who left in 2025 by the cutoff date.

Jordan was found to only have 14 of her pre-service hours in her file. She is required to complete a minimum of 16 of the 24 hours within 90 days of hire. Her documentation on file shows she is beyond that timeframe. She must complete 2 more hours by the date noted to comply by.

There were several cleaning products, including bleach based scrubbing chemicals located under the unlocked sink in the pre-k room. They were moved to an area outside of the reach of children during the inspection.

There is no visible posting on the parent board in the entrance or in the classrooms that contains a current list of employees.

A staff person was present at the operation without a letter of eligibility.
