The play structure on the school-age playground had missing screws at the top of the slide. The post to the metal gate is broken and the cement foundation is cracked and unstable. The funnel ball post is also loose and unstable. The platform on the pre-k playground structure contains a cracked post.

1 caregiver did not have documentation showing they meet the minimum education requirements. The education documents for 3 caregivers were written in a foreign language and have not been translated into English.

Observed a child sleeping in a baby bouncer during the inspection. Note: The provider removed the child from the bouncer and placed them in their assigned crib.
