Both transportation vehicle were missing the thermometer. CORRECTED AT INSPECTION: Thermometer was added at the time of inspection.

746.4907(d) - Installation and Maintenance of Loose-fill Surfacing - Proper Depth Maintained

The loose-fill is not being maintained as the trap is visible and accessible to the children as a tripping hazard.

746.1609 - Maximum Group Size 13 or More Children

In the 18m to 24m classroom during nap time, 10 children were observed to be with one caregiver, bringing the room out of ratio by 1 child. 8 children were observed to be awake.

746.4805(a) - Measurement of Use Zone for Slides - in Front of Access and on Sides

In the 2 to 3 yr old play area, the stationary equipment with a slide was observed to not have the required 6 feet requirement that extends out from the front of the slide and the sides.

746.3701 - Safety - Areas Free From Hazards

The following hazards were observed outside in the children's play areas: gate with latch is to low and in access to the children that leads to the street, two wooden shelves that are not mounted and pose has a tipping/fall hazard, and three sewer capped pipes that are exposed from the ground and posing a tripping hazard.

746.3415(4) - Employee Handwashing After Diapering a Child

The caregiver and child did not wash hands after diapering. This was corrected once the caregiver returned to wash hands and those of the child.

746.1609 - Maximum Group Size 13 or More Children

The classroom had 17 children bringing the room out of ratio by 2 children. This was corrected when two children were taken to another classroom.

746.5101(b) - Annual Fire Inspection - Conducted by Fire Marshal

The annual fire inspection had expired.

746.3401(a) - Annual Sanitation Inspection

The annual health inspection had expired.

745.621(a)(2) - AP Initial background checks submitted - At the time you hire someone

Inspector observed one staff present and in a classroom with unsupervised access to children between the ages of 13 months-17 months. The staff did not have an initial background check submitted and had not received the results from the background check or FBI fingerprints.

746.301(2) - Notification of Change in Space

At the time of inspection, the operation had switched two infant classrooms and two toddler classrooms. No amendment was submitted so that the space could be reviewed.
