During the inspection the infant room has a fan on the shelf not mounted.

746.3701 - Safety - Areas Free From Hazards

During the inspection an inflatable pool in the playground area had standing water inside. The fence for the playground located in the back behind the daycare has loose boards. Corrected at inspection: The water was removed from the inflatable pool.

746.1205(a)(2) - Responsibilities of Caregivers - Know number of children responsible for

Caregivers on the playground with the mixed age group did not provide the correct number of children they were supervising. This was corrected when the caregiver had the children line up and do a head count.

746.1205(a)(3) - Responsibilities of Caregivers - Know children's names and ages

A caregiver in the mixed age group on the playground could not tell me how many of each age was on the playground. This was corrected when the director explained that bright wheel can be used to inform licensing of the children's ages and caregiver provided ages.

746.901(4) - Required Personnel Records - Required Affidavit (Form 2985)

Four staff files were reviewed and three of the files did not have a notarized affidavit available for review.

746.605(11) - Required Admission Information - Emergency Medical Authorization

Eight children's records were reviewed and one file did not have emergency care authorization or facility information.

746.1205(a)(3) - Responsibilities of Caregivers - Know children's names and ages

The caregiver in the infant classroom was not able to tell me the name or age of three children in the classroom.

746.1601 - Child/ Caregiver Ratio - 13 or More Children

There was one caregiver in the infant classroom SAG 12-17 months old with 11 children. In the toddler classroom SAG 4 years old there were 21 children with one caregiver when the second caregiver had to step out the classroom.

746.605(6) - Required Admission Information - Emergency Contact

Eight children's records were reviewed. ONe child's record did not include the emergency contacts address.

746.5305 - Mounting of Fire Extinguishers

I observed an unmounted fire extinguisher on the second floor of the operation.

746.5537(a)(1) - Electric or Battery-Operated Carbon Monoxide Detector-Install new Battery in Each CO Detector Annually

CCR staff tried to test the CO detectors on the first and second floor and they did not work.

746.3407 - Maintenance of Building, Grounds and Equipment

The playground contains wooden climbing equipment that has pieces of wood that are deteriorating, nails protruding, pllastic covering that were cracked/broken and moving boards that were not secure. There was also trash and debris that CRR staff requested to be removed.

746.2405(4) - Infant Care Area Furnishings and Equipment - Hand-washing Sink

I observed dirty bottles in the sink that is desinated for hand washing in the infant classroom.

746.4307 - Fenced Outdoor Activity Space -Exits Required

The playground at the front of the operation only has one exit.

746.4907(a)(1) - Installation and Maintenance of Loose-fill Surfacing - Six Inches if Equipment 5 Ft or Less

There are three playground areas on the operations property observed with less than 6 inches of loose fill.
