Children's records were reviewed during the inspection. Several children's records were missing health statements, immunization records, and parent rights information.

746.1205(a)(4) - Supervision ensures each child?s safety, well-being, including physical proximity and auditory or visual awareness of each child?s on going activity

During the course of the DFPS investigation it was determined that a child was not properly supervised when the child went back into the classroom while the rest of the class was still on the playground.

During the course of the DFPS investigation it was determined that day care staff yelled at a child in care.

746.1325(a)(1) - Documented annual training - Employee prior to accompanying a child to a swimming pool

Evidence gathered during this investigation shows the caregivers did not have the required water safety training prior to taking a group of children swimming at the city pool.

746.3417(3) - Children Handwashing After Toileting or Diapering

During the unannounced inspection, children did not wash their hands after going to the bathroom.
