746.1309(e) - Documented Annual Training- Required Content

One out four employee records were missing the following required trainings hazardous materials, allergic reactions, medication administration, building safety, emergency preparedness, and communicable disease for the annual year 2/18/25-2/18/26.

746.2421(a) - Written Feeding Instructions - Signed and Dated by Parent or Health-Care Professional

There was not an infant feeding plan completed for two of the infants that still do not eat table food.

746.2426(a)(1) - Infant May Not Sleep in a Restrictive Device

During the inspection, a 3-month old infant was observed sleeping on a restrictive device. Inspector asked caregiver to move child to a crib and this was completed. Later in the same inspection, the same child was observed sleeping on a bouncer.

746.2803(3) - Discipline - Teaches Acceptable Behavior and Self-control

During the inspection, it was observed that a caregiver was using high chairs to keep three children seated over 15 minutes after eating as the caregiver mentioned that the children are unable to remain seated during activities.

746.5205(4) - Documentation of Drills

Last fire drill practiced was on 07/12/2025. The fire drill practiced in August was not documented in the form.

746.3407(12) - Mark Cleaning Supplies, Toxic Materials; Keep Separate from Food and Inaccessible to Children

In two different restrooms accessible to children, there were cleaning products and shaving creams stored under the sinks, which lacked safety locks.

It was observed that the television was turned on showing cartoons in a classroom with 4 infants ranging from 6 months up to 20 months. Please note: Television was turned off during inspection.
