Six of ten children files reviewed did not have a health statement on file.

746.613(b) - Immunization Records, Exceptions, and Exemptions Current

Three out of ten children's files reviewed did not have up-to-date immunization records available in the file.

746.1601 - Child/ Caregiver Ratio - 13 or More Children

While reviewing video footage provided by the operation, it was determined that there were four different classrooms out of ratios on two separate days, November 5 and November 6, 2025. The first classroom was a children's specified age group, three-year-olds; there were 17 children supervised by one caregiver, and out of ratio by 2 children. In the second classroom, the specified age group was two-year-olds with 12 children and one caregiver, out of ratio by one child. The third classroom is children children-specific age group,12 to 18 months, there were 7 children to one caregiver, out of ratio by 2 children. The fourth classroom is for children of a specific age group of 0 to 12 months, had 5 children with one caregiver, an over ratio of one child, and then during the day, there were two caregivers with 11 children over ratio by one child.

746.1601 - Child/ Caregiver Ratio - 13 or More Children

While reviewing video footage provided by the operation, it was determined that there were two classrooms on two different dates that were left out of ratio. In the Preschool A room with three-year-olds had 19 children with one caregiver. Young Preschool C with two-year-olds had 13 children with one caregiver. Young Preschool C with two-year-olds on a different day had 30 children with two caregivers. The classrooms were observed to be out of ratio for over 15 minutes.

The operation does not have a current fire inspection. The last fire inspection was conducted 4/8/24.

746.5209(c) - No Blocked or Locked Doors as Exits

In 2 classrooms, napping children were blocking the fire exit.

746.1205(a)(2) - Responsibilities of Caregivers - Know number of children responsible for

A caregiver in the toddler room did not know the number of the children in their care. Note: This was corrected at inspection when the caregiver counted again and gave the correct count.

At time of inspection an infant teacher was not able to provide ages for children in care.

At the time of inspection a caregiver in an infant room stated there were seven children in care when there were eight. - This was corrected during the inspection.

At time of inspection an infant was observed sleeping in a restrictive device. - This was corrected during inspection.

746.1609 - Maximum Group Size 13 or More Children

The infant classroom was out of ratio as 11 infants were observed to two caregivers. The correct ratio for the 0-11m age group is 10 infants to two caregivers. Note: This was corrected at inspection when one of the children were moved to another classroom.
