746.1205(a)(4) - Supervision ensures each child?s safety, well-being, including physical proximity and auditory or visual awareness of each child?s on going activity

Based on interviews and videos, two staff did not make a name to face count or a thorough sweep of the playground when transitioning the children from outside to the inside of the building resulting in a child being left on the toddler playground for 1 minute and 3 seconds. The PreK and the afterschool areas were being utilized at this time, but staff were unaware of the child that was left until the child was noticed by a caregiver.

746.305(a)(6) - Report Situation Placing Children at Risk

The Licensing intake line was not called when the operation was made aware of allegations that two caregivers left a child on the playground without conducting a name to face count.

746.1205(a)(2) - Responsibilities of Caregivers - Know number of children responsible for

At the time of the inspection, the caregiver in the PreK room did not know how many children were in her care. She stated there were 16 children in her care when there were 17.This was corrected when the caregiver recounted the children on the Ipad.

The Preschool teacher stated there were 15 children in care and changed the number to 14. After verifying on the Ipad there were only 13 children in care.

Based on the interviews and video the operation staff did not do a name to face count or a sweep of the classroom when transitioning the children from indoors to outside resulting in a child being left unattended in the room for approximately 8 minutes.

746.2805(3) - Prohibited Punishments - Grabbing and Pulling

Based on the video, a teacher was seen holding a child?s hand and dragging the child while still walking on the playground even after the child drops to the ground on his bottom.

Operation staff did not do a name to face count or a sweep of the classroom when transitioning from indoors to outside resulting in a child being left unattended in the room for approximately 10 minutes.

746.2419(6) - Requirements for Feeding Infants - Label Bottles/Cups

More than one bottle between the two infant rooms were missing the last initial of the infant's name that is required. This was corrected when the last name initial was added to the bottles.

746.3819 - Food Allergy Emergency Plan Signed by Parents and Health Care Professional

Two food allergy emergency plans did not have a parent signature.

746.3407 - Maintenance of Building, Grounds and Equipment

The plastic containers in two rooms that hold toys were damaged, leaving rough edges that could pose a risk to children. This was corrected when the containers were removed. The bathroom doorknobs in the afterschool room were broken and sharp to the touch. This was accessible to children while opening the bathroom doors. This was temporarily corrected by covering the knobs with tape until the replacement knobs arrive.
