746.3701 - Safety - Areas Free From Hazards

The bolts anchoring the trim around a tree were observed to be loose and coming out of the ground. This was corrected at inspection when the director removed them.

746.1205(a)(2) - Responsibilities of Caregivers - Know number of children responsible for

Caregiver reported there were 13 children in a Toddler classroom when there were 12 children in care, and another caregiver reported there were 12 children in a Preschool classroom when there were 13 children in care. Note: This was corrected at inspection when staff confirmed the correct number of children in their care.

746.3417(3) - Children Handwashing After Toileting or Diapering

In two classrooms, the children did not wash their hands after getting their diaper changed.

746.3415(4) - Employee Handwashing After Diapering a Child

One caregiver was observed leaving their gloves on to change diapers without washing their hands. The second caregiver was observed changing gloves in between diapering and did not wash their hands before putting on new gloves.

746.5305 - Mounting of Fire Extinguishers

During the inspection, the door handle for the fire extinguisher was broken off, leaving the fire extinguisher not immediately accessible.

During the inspection, there were no up-to-date lesson plans posted in the classrooms.

746.3505(f) - Diapering Procedures-Soiled Diapers Kept in a Sanitary Manner

During the walkthrough, there was a garbage can without a lid observed in the restroom. There were dirty diapers observed in the can, accessible to children.

During the inspection, there were nine classrooms without current menus posted. Note: this was corrected at inspection when the current menus were printed and posted in all classrooms

During the records review, there were two children missing the required Health care Provider Statement.

At time of inspection, a PreK caregiver stated they had eighteen children in class when they had fourteen children.
