746.1203(5) - Responsibilities of Caregivers - Interact positively with children

During the course of the DFPS investigation, it was determined that caregiver did not interact with a child in a positive manner resulting in a scratch on child's shoulder.

746.1205(a)(4) - Supervision ensures each child?s safety, well-being, including physical proximity and auditory or visual awareness of each child?s on going activity

During the investigation, it was determined that two caregivers left a child unattended for approximately eleven minutes.

746.307(b)(4) - Parental Communication - Situation that Placed a Child at Risk

The operation informed a parent that their child had been left unattended in a classroom the day after the incident occurred.

There was a Big Joe bean bag used by staff in the infnat II classroom Note, this was corrected at inspection when the bean bag was removed from the classroom.

746.3701(8) - Safety-Hinged Lid Storage Containers Safe

There were two storage containers on the playground that did not have ventilation holes. One of the containers had a small latch and the other did not have the lid support arms. Note, this was corrected when the storage containers were removed from the playground.

746.3805(a)(1) - Administering Medication - According to Label Directions

A child was not administered their inhaler as prescribed on the medication box while in care at the operation.

746.901(4) - Required Personnel Records - Required Affidavit (Form 2985)

At time of inspection a notarized affidavit did not have the notarized seal on affidavit.

746.603(a)(4) - Children's Records - Immunizations

At time of inspection, two out of ten children files did not have immunization records. One out of ten children files did not have an updated immunization record.

746.605(13) - Required Admission Information - Special Needs Statement

At time of inspection, eight out of ten children's files did not have completed special needs section in the admission form.

746.1309(a) - Documented Annual Training Area - 24 Hours Required

At time of inspection nine out of ten staff files did not have completed 24 annual training hours.

746.1315(a) - Each Caregiver and the Director Must Have Current Training in pediatric first aid with rescue breathing and choking.

At time of inspection one out of ten staff files did not have a current CPR/First Aid certification.

746.3407 - Maintenance of Building, Grounds and Equipment

At time of inspection, there was a loose doorknob in a prek classroom. At time of inspection a toddler class ceiling was missing a ceiling tile missing.

746.2415(b) - AP Cribs - Only Snug Fitting Sheet and Crib Mattress Cover Allowed if Infant Younger than 12 Months of Age

At time of inspection, a toy was observed inside a crib with an eight month child sleeping. Corrected at inspection.
