This standard was evaluated as part of a follow up and found deficient. A child was left unattended on the playground.

746.3503(c)(1) - Diaper Changing Equipment- If Above Floor Level Must Use Safety Mechanism

On the four diaper changing tables above floor level, the diaper changing mats are flat on a counter. One side of the counters does not have a raised side.

746.3503(c)(2) - Diaper Changing Equipment - If Above Floor Level Must Keep Hand on Child

This standard was evaluated as part of the investigation and was found to be deficient. Through interviews with staff, it was determined that a caregiver did not keep their hand on an infant who was on the diapering table.

746.3503(c)(3) - Diaper Changing Equipment - If Above Floor Level Caregiver Must Face Child and Be Within Arm's Length of Child

This standard was evaluated as part of the investigation and was found to be deficient. Through interviews with caregivers, it was determined that a caregiver was not facing an infant and was more than an arm's length away from the infant when the infant fell off a diaper table and received a minor injury.

746.5015(a) - Safety Requirements for Sprinkler Play - Not Used On or Near Hard, Slippery Surface

During the investigation it was found that a group of children were participating in sprinkler play on the concrete basketball court of the facility.

746.4805(c) - Measurement of Use Zone for Slides - Greater than Six Feet Exit of Slide

On the school age playground, there was one slide that was 6 feet 8 inches in height; the use zone for the slide was 5 feet 4 inches. The use zone could not extend beyond 5 feet 4 inches because a wooden barrier was blocking the use zone from extending further.

746.3401(a) - Annual Sanitation Inspection

The operation did not have a current health inspection under the new ownership.

746.4907(a)(1) - Installation and Maintenance of Loose-fill Surfacing - Six Inches if Equipment 5 Ft or Less

The swings, and slide and entrances/exits to the slide structure in the Preschool playground had mulch that was 1-4 inches in depth in their use zones. This was corrected at inspection when the operation put up barriers to prevent children from using the equipment.

746.5101(a) - Annual Fire Inspection - Before Initial Permit Issued and Every 12 Months

The operation did not have a current fire inspection under the new ownership.

746.4907(a)(2) - Installation and Maintenance of Loose-fill Surfacing - Nine Inches if Equipment Greater Than 5 Ft.

On the school age playground, the mulch was 1-5 inches in depth in the use zones for the tire swing, ladder to equipment, slide, and two sets of monkey bars. This was corrected at inspection when the operation put up barriers to prevent children from using the equipment.
