A shared space plan was not available outlining how the operation would maintain supervision and the safety of the children in care when another independent after school group joined the operation in the cafeteria.

744.1205(a)(4) - Supervision ensures each child?s safety, well-being, including physical proximity and auditory or visual awareness of each child?s on going activity

A caregiver was observed sitting on a set of stairs while children were playing on the playground.

744.2105(8) - Prohibited Punishments - Withholding Active Play

Caregivers were overheard telling children they would "owe them 5 minutes" for not following directions or stay inside instead of going outside during their playground time for continuing to roll around the room on the rolling chairs after being asked to stop.

744.2507(9) - Sanitize Table Tops, Furniture, Equipment

Prior to eating snack, surfaces children were eating on top of had not been cleaned or sanitized. Children had been sitting on top of a bench in the room prior to being served snack and then placed their snack directly on top of the bench area while eating. There was not a cleaning or sanitizing solution available.

744.1203(3)(D) - Personal use of electronic devices, such as cell phones, MP3 players, tablets, and video games

A caregiver was observed scrolling on their phone while children were playing outside on the playground.

744.2419(6) - Food Service and Preparation Requirement - Use Sanitary Holders

Children were observed eating snack at tables, a coffee table and on top of a bench in the room. Children were placing their snack directly on top of the surfaces and did not have a plate or paper napkin provided.
