A caregiver aggressively grabbed/pulled a child during a disciplinary incident at the facility. The child was uninjured. The caregiver responsible was terminated for her actions. Remaining staff had completed additional trainings on positive discipline/guidance to mitigate risk.

One caregiver was lacking current training for children under 24 months of age. This caregiver was working in the 12-18 month old room.

Two children's records were lacking the address for the emergency contact.

746.3701 - Safety - Areas Free From Hazards

The following safety hazards were observed on the playground: *A space under one of the buildings was lacking the barrier to make this area inaccessible. The building was located on the playground. *A piece of metal was sticking out of the ground on the back playground. Pictures were taken.

746.1316(a) - Transportation Safety Training -Director and Employee Complete 2 Hours Annually

The Director was lacking current transportation training. The operation provides transportation.

746.5625(b) - Electronic Child Safety Alarm - installed and maintained according to manufacturer's instructions

The electronic safety alarm was not installed on the operation's bus as required. The director will ensure that the safety alarm is installed by the compliance date.
