One medication stored was observed to have expired in September 2025. NOTE: This was corrected at the time of inspection when it was disposed.

746.501(a)(20) - Written Operational Policies - Parent Rights

The written operational policy was currently missing reviewing records, comply with court orders, facility written records, and retaliatory actions.

746.201(11) - AP Ensure total number of children both at & away from center within capacity

It was determined that the center is over capacity. The center current capacity is 24 and the center had 25 children on 10/14/2025 and 26 children on 10/22/2025.

746.301(2) - Notification of Change in Space

The operation is using the 3 year old playground that has not been inspected and approved by CCR. This was corrected when CCR inspector measured the playground on 10/22/2025. The operation did not notify CCR that they have added 3 years old to their services and added a room for the 3 year olds. The permit indicates ages infants to two years olds.

746.4751(a) - Maintenance Procedures for Active Play - Inspect Daily

Based on information gathered, staff member did not check the big playground prior to the children using the playground.

746.1205(a)(4) - Supervision ensures each child?s safety, well-being, including physical proximity and auditory or visual awareness of each child?s on going activity

Based upon information gathered and interviews conducted it was determined a child was found in a secured gated playground unsupervised.

746.901(4) - Required Personnel Records - Required Affidavit (Form 2985)

One of the four staff files was missing; affidavit 2985, training hours, orientation statement and education information
