746.1011(a) - Director present-Minimum 75%

It was found that the director is not present at the operation 30% of the operating time. It was found that the operation is open, with children from 3:15 until 6:00pm Monday- Friday during the school year and the director is present at the operation Monday-Friday at around 5:20 until 6:00pm, which is less then 75% of the operating times.

746.1301(a)(1)(A) - Training Requirements for Employees, Caregivers, and Directors - Orientation

There was no proof of orientation for any of the employee's files that were reviewed.

746.3401(a) - Annual Sanitation Inspection

The operation could not provide proof that a sanitation inspection was completed. The last sanitation inspection was 9/27/2024

746.605(11) - Required Admission Information - Emergency Medical Authorization

During a review of the children files, there were 4 children who did not have the emergency medicall attention completed.

746.201(1) - Policies Comply with Rules

The operation could not provide me with the operational policies for review during the inspection.

746.901 - Required Personnel Records

Observed one caregivers file that did not have the pre employment affidavit, the notarized affidavit, photo ID, proof of education. Observed another caregivers file that did not have the notarized affidavit or the pre-employment affidavit. There was also no proof of education, photo ID. Both files only had 8 hours of pre service training.

746.5307(b) - Fire Extinguishers Serviced

Observed the fire extinguisher had not been serviced and was not tagged. The fire extinguisher on the van has also not been serviced within the last year.

746.5205(1) - Fire Drill Monthly - Exit in 3 Minutes

There were no emergency drills documented for review. A fire drill was completed during the inspection.
