During inspection, an electrical outlet in a toddler class was not covered. This was corrected during inspection.

At time of inspection a provisional caregiver was left alone with children, violating a condition on background. Note: This was corrected at inspection as caregiver was then removed from class.

The Operational handbook didn't have a written Safe Sleep policy for infants from birth through 12 months old available for review. NOTE: Corrected at inspection.

746.5202(1) - Emergency Preparedness Plan - Evacuation, relocation, and sheltering/lock-down procedures

The emergency preparedness plan didn't specify how children will be evacuated for children under 24 months of age, who have limited mobility, or who otherwise may need assistance in an emergency, such as children who have mental, visual, or hearing impairments.

One of the four employees was missing form 2985 at the time of inspection.

One of the four children files was missing authorization information, emergency contact address, school age permission and school information. One of the four children were missing special care statement. One of the four children was missing food allergy information.
