A four year old child did not use a booster seat when being transported to school.

It was observed the infant's room did not have adequate lighting to allow visual supervision of the children in care.

During the inspection there was observed a two year old child in the infant's room. The youngest child in the room was five months old. There was a gap of age of 19 months old. It was corrected at the inspection time when the child was removed from the infant's room.

The diaper changing pad was observed with open areas exposed to germs.
