A vehicle utilized for transporting children lacked the necessary information needed to identify the center.

745.621(a)(2) - AP Initial background checks submitted - At the time you hire someone

During an inspection, a staff member had direct access to children without a background check. This was corrected during the inspection as the staff member left the premises.

746.2805(1) - Prohibited Punishments - Corporal Punishment

During the course of the investigation, it was determined that caregivers thumped multiple children as a means of disciplinary action.

746.305(a)(6) - Report Situation Placing Children at Risk

The standard was evaluated during an investigation and was determined to be deficient. The operation failed to notify the childcare regulation within the required 48 hours of becoming aware that their daycare vehicle was involved in an accident while children were on board.

746.5607(a) - Child Passenger Safety Seat System - Must Use for Each Child in When Transporting and be Properly Installed

The standard was assessed during an investigation and found to be in deficit. It was discovered that children were placed in seats with non-working seatbelts. upon inspecting the vehicle it was found that three seatbelts did not function.

746.5205(1) - Fire Drill Monthly - Exit in 3 Minutes

The center had not conducted a fire drill since January 2025. This was corrected during inspection. A fire drill was conducted during the inspection.

746.5205(4) - Documentation of Drills

The center did not have any documentation available to review regarding the drills.

746.2428 - No Swaddling of Infant Unless Completed Sleep Exception Form on File

During a walkthrough an infant was observed inside their crib swaddled. This was corrected during the inspection and the infant arm were placed outside.

Five children files reviewed did not have the emergency medical care infomation.
