One child with a special care need diagnosed by a doctor was missing the statement including the cognitive or mental condition that may warrant prevention or intervention while the child is in care. This was corrected at inspection when the designee/parent completed this section in the child's admission paperwork.

The name, address, and telephone number of the child's physician or an emergency-care facility was missing from one child's record. This was corrected at inspection when the designee/ parent of the child filled out this section of the admission paperwork.

The director and designee were missing the Pre-Employment Affidavits for Applicants for Employment from their personnel files.
