746.3701 - Safety - Areas Free From Hazards

In 3c classroom an ant and roach spray can was found in a cabinet accessible to children as the door latch was broken. NOTE: This was corrected at inspection as the spray can was taken out of the classroom and disposed.

746.613(b) - Immunization Records, Exceptions, and Exemptions Current

Four of the ten children records evaluated were missing shots at the time of the inspection.

746.801(10) - Required Records Maintained and Made Available - Safety Documentation for Emergency Drills, Fire Extinguishers, & Smoke Detectors

No documentation for fire extinguishers, carbon monoxide detectors, and smoke detectors were available for review. In addition to March and June 2025 fire drills documented. NOTE: Fire drills were corrected as they were noted on their calendar.

746.5307(b) - Fire Extinguishers Serviced

Nine of the fire extinguishers were observed with no tags or 1/2024 tags.

746.3407 - Maintenance of Building, Grounds and Equipment

At time of inspection, a refrigerator in a toddler class was observed to have debris. This was corrected at inspection.

A time of inspection, one monthly feeding schedule was last updated on November 2024.

746.605(12) - Required Admission Information - Special Needs Statement

At time of inspection three out of eight children files had special needs section blank.

746.4503(a) - Individual Cot, Bed, or Mat for Each Walking Child Through 4 Years of Age

At time of inspection, two mats were observed torn in a PreK classroom.
