It was observed that there were several areas on the three year old playground has the underground black nylo exposed. This can pose a tripping hazards to children in care.

It was observed that mulch on the three years old playground is less than six inches. More loose fill surfacing is needed.

The van is missing a fire extinguisher.

There was not a current CPCS form. This was corrected at inspection when the director signed a new form.

The van was missing a first aid kit. This was corrected at inspection when the director put a kit in the van.

746.401(9) - Posting Requirements - List entitled "Current Employees"--sized at least 81/2 X 11 inches, printed legibly, include employee first and last name

The employee list was outdated. It was corrected at inspection when the director updated and reposted the list.

746.5617(a)(6) - Required Equipment in Vehicles - First-aid Kit

The van was missing two items from the first aid kit. This was corrected at inspection when the missing items were added.

One child file is missing a parent signature and date.

746.1403(a)(2)(A) - Training requirements for substitutes, volunteers, and contractors-Pre-service training

The center had high school interns working here without the required training. This was corrected at inspection as those interns are no longer here and the requirements were discussed with the director.

746.4601(7) - Safety Requirements for Active Play Equipment - No Pinch, Crush, or Shear Points

The playground had a sharp, rusty, loose metal barrier around two pieces of playground equipment. This was corrected at inspection when the director removed the barrier and disposed of it.

746.1301(a)(3)(A) - Training Requirements for Employees, Caregivers, and Directors - Pediatric First Aid with Rescue Breathing

3 of 4 teachers within their first 90 days of employment and without first aid training were observed with unsupervised access to children.
