It was determined during an investigation a caregiver hit a child on the arm as a form of discipline.

Five medications observed not labeled with date brought to operation. Five medications required on food allergy plan were not available on site at time of inspection. Corrected at inspection when operation labeled all medications with date received. Corrected at inspection when medications were provided.

Infant observed wearing an necklace. Corrected at inspection when caregiver removed necklace and brought to office.
