A caregiver was observed walking out of a room while the children were sleeping making the caregiver unable to intervene in the event of an emergency, No other caregivers were in the room.

During the inspection, a child under the age of two was observed having screentime.

The diaper changing mat in the infant classroom was observed to have rips and tares.

One of the personnel files reviewed was missing a preemployment affidavit, cpr and first aid training, and orientation.

The annual fire inspection was not conducted. The last fire inspection was in April of 2024.

The outside playground area had a pile of leaves in the corner of the playground and broken toys where the children had access to.

An infant was observed sleeping in a bouncy chair. This was corrected at the inspection when the caregiver moved the child to his assigned crib.

There was not enough unitary surfacing installed under the slides.

An infant was observed swaddled sleeping in a crib.

The caregiver was found to be outside of two classrooms while four toddlers were napping in one room and two infants were napping in a seperate room.

An infant was observed sleeping with a cup. This was corrected at the inspection when the cup was removed from the infants crib.

There was only one caregiver in the center during nap time with 5 children.
