Based on the preponderance of the evidence, a child accidentally spilled hot soup onto their leg, causing a second-degree burn as the soup was not served in a safe manner.

746.1105(3) - Minimum Qualifications Employees and Caregivers - Affidavit (Form 2985)

Five caregiver files were missing a notarized affidavit.

745.641 - AP Background check results - Must receive notification prior to allowing subject to be present at your operation

During inspection, a caregiver was observed to be supervising children without cleared background checks, to include pending fingerprints and pending national sex offender registry checks. Caregiver was allowed to be present before receiving notification from the criminal background check unit that the caregiver was eligible. This was corrected when this caregiver was removed from the operation. Caregiver's fingerprinting appointmnet is scheduled for 5/30/2025.
