746.5205(4) - Documentation of Drills

During the unannounced inspection, emergency drills were reviewed. There is no documentation of the required drills.

746.3809 - Medication No Longer Used

Medication as observed during the unannounced inspection. One child had an expired Epi-pen.

746.3819 - Food Allergy Emergency Plan Signed by Parents and Health Care Professional

During the unannounced inspection, food allergy records were reviewed. Two children with diagnosed food allergies that require an Epi-pen did not have the Food Allergy Emergency Plan on file.

746.5205(1) - Fire Drill Monthly - Exit in 3 Minutes

During the unannounced inspection, emergency preparedness procedures were reviewed. The operation has not conducted the monthly fire drills as required.

746.2419(6) - Requirements for Feeding Infants - Label Bottles/Cups

Based on information obtained during the investigation, it was determined that a child's bottle was not labeled correctly and had not been properly observed when received. This error led to a child receiving the wrong bottle which contained breast milk.

745.665 - If a subject of a background check at your operation has been arrested or charged with a crime, you must report it to Licensing immediately

During the course of the investigation, it was determined that an individual at the operation who is required to have a background check was arrested. The operation did not notify Child Care Regulation as required.

746.901 - Required Personnel Records

9 personnel files were reviewed during the inspection. 5 of the personnel files were missing required records.

745.621(a)(2) - AP Initial background checks submitted - At the time you hire someone

A caregiver who was present during the inspection had recently transfered from another child care operation within the same company. A background check had not been submitted for this operation and was inactivated at the previous operation. The director submitted the background check request in the parent portal during the inspection. The caregiver will not return to the operation until an eligibility determination is received from the CBCU.

746.3407(9) - Floors, ceilings and Walls in Good Repair and Clean

A large hole in the classroom wall was observed during the inspection. It is located in the children's library area of the classroom and is accessible to children creating a safety risk.

746.5617(a)(5) - Required Equipment in Vehicles - Fire Extinguisher

Three vans being used to transport children were observed to not have a fire extinguisher. These vans are rentals that are being used temporarily. This deficiency was corrected during the inspection by moving fire extinguishers that were located on buses that are no longer in use.
