746.2805(1) - Prohibited Punishments - Corporal Punishment

Based on information obtained during the course of the investigation, there is sufficient evidence to support the allegations that a caregiver used prohibited punishment when she hit a child on the face and called him a baby.

746.1601 - Child/ Caregiver Ratio - 13 or More Children

Based on information obtained during the investigation, it was determined that the infant classroom was over ratio for approximately one hour on a daily basis.

746.3417(3) - Children Handwashing After Toileting or Diapering

During the unannounced inspection, diaper changing procedures were observed in two classrooms. In both classrooms, the caregiver was observed to allow the child to return play activities without washing their hands. This creates unsanitary conditions and places children at risk of illness.

746.4503(b) - Cots, Beds, Mats Labeled With Child's Name

Based on information obtained during the investigation, this standard is deficient. One classroom was identified where the nap mats were not labeled with the child's name and no other identification process was used. Sheets were placed on the nap mats and were not changed during the week unless soiled. Children were allowed to sleep on any mat each day, and therefore, were allowed to sleep on sheets that had been previously used by other children.

746.3417(1) - Children Handwashing Before Eating

During the unannounced inspection, children were observed not washing their hands. In one classroom, the soap dispenser at the children's handwashing sink was observed to be empty.

746.1301(a)(2)(A) - Training Requirements for Employees, Caregivers, and Directors - Pre-Service Training

During the inspection, staff records were reviewed. It was determined that the need for pre-service training is not being consistently evaluated and pre-service training is not being conducted prior to a caregiver being placed with children.

746.3505(e) - Diapering Procedures - Sanitizing Diaper Changing Area

Diaper changing procedures were observed during the unannounced inspection. In one classroom, there was no barrier (such as a paper liner) placed between the child and the diaper changing surface. After the child's diaper was changed, there was no cleaning and sanitizing of the surface before placing another child on the changing table. This creates an unsanitary environment which places children as serious risk of illness.

746.1301(a)(1)(A) - Training Requirements for Employees, Caregivers, and Directors - Orientation

During the inspection, staff records were reviewed. It was determined that orientation training is not being conducted consistently with newly hired staff members prior to placing them in ratio with children.

Information regarding the type of water activities that the children in care can participate in while in care needs to be added.

A child in care with a diagnosed severe food allergy that requires an epi-pen needs a emergency allergy plan signed by the child's health care professional.
