During the inspection, diaper cream was observed inside a child storage area. It was noted that the diaper cream was accessible to toddlers in the classroom. This was corrected during the inspection, and the item was removed.

746.3317 - Food Service and Preparation Requirement - Safe and Sanitary

During an inspection of the daycare kitchen, it was observed that food items were present in the handwashing sink.

746.2419(6) - Requirements for Feeding Infants - Label Bottles/Cups

Two bottles in the infant room were not labeled during the inspection. This was corrected at inspection Staff labeled bottles.

746.3407(8) - Garbage Inaccessible to Children; Maintained in Sanitary Manner

In the toddler classroom, there were two trash cans containing garbage that was easily accessible to the children. This was corrected during the inspection the trash was removed.

746.3407 - Maintenance of Building, Grounds and Equipment

A crib mattress was observed in a damaged condition, with exposed internal padding and a visible metal support component. The mattress did not provide a safe, intact sleep surface for children. This was corrected at inspection, and the mattress was exchanged for another.

746.3701 - Safety - Areas Free From Hazards

During the inspection, a television cord was observed hanging down the wall within reach of children in the one-year-old classroom. The exposed cord posed a potential strangulation and electrical hazard to children in care. The operation did not ensure that the area was free from hazards accessible to children.

746.4813(a) - Measurement of Use Zone for Rotating and Rocking Equipment (Sitting)

During the inspection, I observed a spring-mounted school bus" climber located less than six feet from the chain-link fence. This was corrected at inspection. The outdoor equipment was moved to meet use zone requirements.

746.3407(9) - Floors, ceilings and Walls in Good Repair and Clean

The infant classroom door is missing screws, making it difficult to open and creating a safety hazard.

746.1109(a)(1) - Caregiver Qualification Age Exemption with HS Diploma/GED- Not in Charge of Group

A caregiver who was not of legal age was responsible for supervising a group of children alone.

746.1315(a) - Each Caregiver and the Director Must Have Current Training in pediatric first aid with rescue breathing and choking.

Caregivers who were supervising children alone did not have their current certification.

746.5617(a)(1) - Required Equipment in Vehicles - List of Children

The operation did not have a list of children being transported to and from school.

746.3417(1) - Children Handwashing Before Eating

Children were observed being fed before washing their hands. This was corrected during the inspection afte addressing the concerns with staff.

746.4503(a) - Individual Cot, Bed, or Mat for Each Walking Child Through 4 Years of Age

During the inspection, it was observed that a child was sleeping in a chair instead of on a mat or cot.

746.305(a)(6) - Report Situation Placing Children at Risk

During the investigation, it was determined that the permit holder and director were made aware of a situation that placed children at risk and failed to notify child care regulation.

746.1507(3) - Determining Child/Caregiver Ratios - Caregiver Skills/Needs of Children

On 6/26/25 during the inspection it was detemened that Staff did not demonstrate the necessary skills to manage the assigned group, and there was no documented training or evaluation of the caregiver's qualifications at the time of inspection. During a Child Care Investigation (CCI), it was determined that the caregiver responsible for eight 12- to 18-month-old children lacked the necessary training and skills to adequately care for this number of children and manage an additional child who was a known runner.

746.1003(1) - Director Responsibilities - Operate in Compliance

The director did not ensure there was enough staff present to manage day-to-day responsibilities in the director's absence. The director left a caregiver in charge of the operation who was also responsible for supervising a group of children and preparing meals.

746.1205(a)(4) - Supervision ensures each child?s safety, well-being, including physical proximity and auditory or visual awareness of each child?s on going activity

During a CCI investigation, it was determined that caregivers did not provide adequate supervision. A child exited the operation and was found by a bystander leaving the child unaccounted for 18 minutes. A total of four children were seen on video unsupervised in two different hallways. Two caregivers were seen on the video and did not intervene to ensure appropriate supervision of the children.

746.1507(3) - Determining Child/Caregiver Ratios - Caregiver Skills/Needs of Children

During a Child Care Investigation (CCI), it was determined that the caregiver responsible for eight 12- to 18-month-old children lacked the necessary training and skills to adequately care for this number of children and manage an additional child who was a known runner.

746.1203(3)(B) - Meal preparation, except when 12 or fewer children are in care

A caregiver responsible for supervising a group of children was preparing lunch for the operation.

746.1203(4) - Provide care consistent with child's habits, interests, strengths, special needs, including any special supervision needs

Staff did not provide adequate care, considering the known needs of a child with a history of running. Failing to account for this behavior put the child at risk. The child got out of the building and was unaccounted for, for 18 minutes.

746.1203(10) - Responsibilities of Caregivers - Share Information with Incoming Caregivers

During a CCI investigation, interviews with staff revealed that a caregiver failed to inform the incoming caregiver that a child was back in their care.

746.3401(a) - Annual Sanitation Inspection

The current health inspection has expired as of 4/3/2024.

746.2409(c) - Safety Requirements for Cribs - Infant Not in Crib with Drop Gate Down

Infants were observed inside cribs with the drop gate open. This was corrected during the inspection.

The permit holder failed to meet several regulatory responsibilities. Issues included failure to report to CCR, inadequate supervision, and poor building conditions. Annual health and fire inspections were not up to date, and safe sleep practices were not followed.
