Two outlets in a toddler room did not have safety covers in place. A surge protector in a Pre-K room did not have safety covers in two openings. This was corrected at inspection when safety covers were installed.

746.2426(a)(1) - Infant May Not Sleep in a Restrictive Device

An infant was observed sleeping in a restrictive device. This was corrected at inspection when the child was removed and placed directly in the crib.

746.4907(a)(1) - Installation and Maintenance of Loose-fill Surfacing - Six Inches if Equipment 5 Ft or Less

The rubber mulch in the use zone of the todder playground was less than six inches. The pea gravel in the use zone of the Pre-K playground was less than six inches.

746.3407(12) - Mark Cleaning Supplies, Toxic Materials; Keep Separate from Food and Inaccessible to Children

A spray bottle in a classroom containing a bleach and water solution was not labeled and a bottle containing water and air freshener solution in the same classroom was mislabeled as bleach and water. This was corrected at inspection with the bottles were properly labeled.
