744.1301(a)(4)(A) - Training Requirements for Employees - Pediatric CPR

During file review there was a staff member with expired CPR First Aid dated 03/2026.

744.2002(9) - Care Requirements 5 or More Hours - No Long Waiting Periods

There was a 15 minute wait period from when the children finished snack and when they were moved to the next activity of centers and gym activity.

744.1601 - Child/ Caregiver Ratio

The class had 27 children in the group and the second class had 26 children which made a total of 53 children. This did not allow the children to be together as a total group and was out of ratio.

744.1507(1) - Determining Child/Caregiver Ratios - Basis

There was a ratio of 1 caregiver to 51 children for the first 15 minutes of the start of the program. This was corrected at the inspection when additional staff arrived at the operation.

744.601(c) - Provide Access to Children's Records - Licensing

The operation did not have access to the children's records for the inspector to review. This was corrected at the inspection when the Program Director arrived at the operation and accessed the children's records.

744.1205(a)(4) - Supervision ensures each child?s safety, well-being, including physical proximity and auditory or visual awareness of each child?s on going activity

Children on the playground were allowed to come inside the building without being escorted to their destination and without the knowledge of the caregiver supervising the group inside knowing the children were now in their care.
