During an inspection at the operation, the young infant classroom was observed to be out of ratio with only one caregiver with 5 infants aged 2 months-9 months in one classroom. This was corrected when one child was moved to the older infant classroom to ensure proper ratio.

Two previous employees' background checks weren't inactivated within the required time frame.

A toddler was observed sitting in a restrictive device due to known destructive behaviors per the assigned caregiver. This was corrected by the caregiver removing the child from the restrictive device.

I observed a child under 12 months sleeping on an activity mat in the infant classroom.

There was an infant without a labeled crib.

I observed a caregiver with a provisionally eligible background check caring for a child alone. This was corrected when the director had another staff member without conditions accompany the caregiver with conditions in the classroom.

The operations fire inspection expired on 10/1/2025.

I observed a 2 month old in a crib with a bib that was not on the child's neck and a pacifier clip on a string with beads. This was corrected when the caregiver removed the items from the crib.

A caregiver provided the incorrect age of a child. This was corrected when the director informed the caregiver the correct age of the child.

Based on information obtained throughout the investigation inappropriate discipline was practiced when a caregiver used physically discipline.

Caregivers did not provide the correct ages of the children they were caring for when asked.

Personnel records did not have employees last day of employment documented.

There were three people in the building without active background checks. One of the people without an active background check left the operation.
