Diaper changing pad was observed to have a rips and tares in the middle and on the side of the diaper changing pad exposing the foam. Corrected at Inspection- the diaper changing pad was replaced during the inspection.

Feeding instructions for three infants was observed to not be updated for the required every thirty days.

Based off the information from the DFPS investigation, it was determined the operation did not follow background conditions for a caregiver that is employed at the operation.

The operation failed to follow user instructions when cleaning. The cleaning product stated the chemical should not be used on food contact surfaces but was used to clean inside the kitchen.
