746.1605(1) - Child/Caregiver Ratio - 18 Months Difference in Age

The director was caring for a group of 9 children of varying ages including and infant in the Pre-k room. The children combined in the group were a 6 month infant, two toddler children, and Six Pre-k children. There was more than an 18 month difference between the youngest and oldest child in the group. The youngest child was 6 months of age and the oldest child was 3 years of age.

746.3420(5) - Hand Sanitizer - Stored Out of Child's Reach When Not in Use

Two bottles of hand sanitizers were observed on top of a children's shelf in the pre-k room. A third bottle of hand sanitizer was observed on top of a children's sink in the toddler room. This posed a safety hazard as the bottles of hand sanitizer were within children's reach. Note: This was addressed with the caregivers and the bottles of hand sanitizer were stored in an area out of children's reach.

746.1311(a) - Director Annual Training 30 Hours Required

The director was missing annual trianing on the following topics: (1) Understanding and using safe sleep practices and preventing sudden infant death syndrome (SIDS); (2) Understanding early childhood brain development; (3) Preventing and responding to emergencies due to food or an allergic reaction; and (4) Handling, storing, and disposing of hazardous materials including compliance with 746.3425 of this chapter (relating to Must caregivers wear gloves when handling blood or bodily fluids containing blood?).

746.1309(a) - Documented Annual Training Area - 24 Hours Required

2 of 4 personnel training records reviewed were missing annual training on several topics. A caregiver was missing annual training for training year 1/8/2025-1/7/2026 on the following topics: (1)Recognizing and preventing shaken baby syndrome and abusive head trauma; (2)Understanding and using safe sleep practices and preventing sudden infant death syndrome (SIDS) ; and (3)Understanding early childhood brain development; (4)Preventing and responding to emergencies due to food or an allergic reaction; (5)Understanding building and physical premises safety, including identification and protection from hazards that can cause bodily injury such as electrical hazards, bodies of water, and vehicular traffic; and (6)Handling, storing, and disposing of hazardous materials including compliance with ?746.3425 of this chapter (relating to Must caregivers wear gloves when handling blood or bodily fluids containing blood?). A second caregiver was missing annual training for training year 4/3/2025-4/2/2026 on the following topics: (1)Recognizing and preventing shaken baby syndrome and abusive head trauma; (2)Understanding and using safe sleep practices and preventing sudden infant death syndrome (SIDS); and (3)Understanding early childhood brain development (1)Emergency preparedness; (2)Preventing and controlling the spread of communicable diseases, including immunizations; (3)Administering medication; (4)Preventing and responding to emergencies due to food or an allergic reaction; (5)Understanding building and physical premises safety, including identification and protection from hazards that can cause bodily injury such as electrical hazards, bodies of water, and vehicular traffic; and (6)Handling, storing, and disposing of hazardous materials including compliance with ?746.3425 of this chapter (relating to Must caregivers wear gloves when handling blood or bodily fluids containing blood?).

746.3407(12) - Mark Cleaning Supplies, Toxic Materials; Keep Separate from Food and Inaccessible to Children

A bottle of Clorox disinfecting wipes and a can of Lysol disinfectant spray were observed on top of a children's shelf in the pre-k room. This posed a safety hazard as the cleaning supplies were withing children's reach. Note: This was addressed with the caregiver during the inspection and the cleaning items were stored in an area out of children's reach.

746.5101(a) - Annual Fire Inspection - Before Initial Permit Issued and Every 12 Months

The childcare operation did not have a current fire inspection. The last fire inspection as conducted on 6/07/23.

746.1301(a)(7)(A) - Training Requirements for Employees, Caregivers, and Directors - Transportation training

Director did not have a current transportation training.

746.1601 - Child/ Caregiver Ratio - 13 or More Children

There was one caregiver caring for 9 infants. The specified age for the group was 12-17 months which allows 5 infants to 1 caregiver. Director was assisting the caregiver but would step-out of the classroom to assist with opening the door and cook duties. Note: The ratios were left in compliance as an additional caregiver was put in the classroom.
