746.2805(3) - Prohibited Punishments - Grabbing and Pulling

A caregiver was observed pulling a child by the arm on the playground in an effort to move them away from another child resulting in the child falling.

746.901(11) - Required Personnel Records- signed dated statement that employee attended abuse/neglect training

Four employees files were reviewed and one employee did not have any documentation regarding if that employee received Orientation.

746.3503(a) - Diaper Changing Equipment - Smooth, Non-absorbent, Easy to Clean Surface

It was observed in the infant classroom that the diaper changing table pad was torn in the middle.

746.3701 - Safety - Areas Free From Hazards

Itt was observed that the fence in the preschool outdoor play area has broken wood boards with holes and gaps at the bottom of the fence of 7 inches. The infant and toddler outdoor play area has gaps under the fence causing large holes due to children digging up the soil. The art easel in the Tulip Patch classroom had a broken tray that was missing a screw causing the tray to be unsecured.

746.2421(b) - Written Feeding Instructions - Review and Update Every 30 Days

Two infants in care did not have an updated feeding schedules for the current month, A review of posted feeding documentation showed that the schedules on file had not been revised to reflect the infant's current feeding needs and routines.

746.3407(12) - Mark Cleaning Supplies, Toxic Materials; Keep Separate from Food and Inaccessible to Children

In the restroom that the children use, there was cleaning chemicals on top of the sink and in the unlocked bottom cabinet. There was a basket on the floor, next to the sink with diaper cream. The care giver's purse was within reach of children. During the inspection, the director removed the cleaning chemicals, the basket with diaper cream from the restroom. and the care giver's purse away from the children.

746.613(b) - Immunization Records, Exceptions, and Exemptions Current

Children files were reviewed and one child did not have an updated immunization record. Child is 17 months of age and there are three immunizations listed on immunization record in child's file.

746.4751(a) - Maintenance Procedures for Active Play - Inspect Daily

The Daily Building Grounds check list has not been documented since January 9, 2026.

746.3415(4) - Employee Handwashing After Diapering a Child

A caregiver was observed placing a child's pacifier in their mouth while wearing gloves they used during a diaper change.

746.3407(8) - Garbage Inaccessible to Children; Maintained in Sanitary Manner

The Daisy Garden room had a trash can without a lid that contained soiled diapers and old food. There was also a plastic bag with soiled diapers hanging on the outside of the classroom door to be taken out to the trash.

A child with a vaccination affidavit on file was missing documentation regarding the child receiving or being exempt from receiving the PCV and HIB vaccinations.

A caregiver hired in April had not completed an orientation.

Three infant feeding plans had not been updated with the parent since, March, September and October.

The carpet in one room and a activity carpet in a second room were stained and in need of cleaning.
