Two children were missing a health statement.

746.1315(b)(1) - CPR Training and Recertification Adhere to Guidelines-skill based

One staff out of seven had a CPR/First aid that did not consist of the use of CPR manikin or hands on skill based instruction with practice.

746.2419(6) - Requirements for Feeding Infants - Label Bottles/Cups

Two bottles in the infant room were not labeled.

746.1205(a)(4) - Supervision ensures each child?s safety, well-being, including physical proximity and auditory or visual awareness of each child?s on going activity

Based on the information gathered during the investigation, it has been determined that child was left unattended, and caregiver was not at a physical proximity which did not allow visual and/or auditory awareness of the child in care.

746.307(b)(4) - Parental Communication - Situation that Placed a Child at Risk

Based on the gatherings during the investigation, it was determined concrete evidence exists to support that the parents of a child in risk were not notified immediately after the incident.

An 11 month old child infant feeding schedule was not available for review.

One out of two crib is not labeled with a child's name or number.

The 18-36 month old classroom did not have enough walking space between the cots.

746.4503(b) - Cots, Beds, Mats Labeled With Child's Name

A mat and several cots were not labeled with name or number during the visit.

746.1205(a)(5) - Responsibilities of Caregivers - Intervene to ensure each child's safety

The 18-36M old classroom had 11 children in care with 1 caregiver. Four of the children were sleeping but the others were crying, standing, and walking around. Caregiver was carrying a child but the other children were following her and myself around the room. The caregiver was unable to intervene to ensure each children in the room was being properly supervised.
