746.1311(d) - Director Annual Training - Caring for children under 24 months

The directors file was reviewed and did not have the required SIDS or Shaken Baby Syndrome training.

746.3401(a) - Annual Sanitation Inspection

The centers last health inspection was conducted on March 6th 2025.

746.2419(6) - Requirements for Feeding Infants - Label Bottles/Cups

Multiple infant bottles were observed to not be labeled.

745.651(1) - Background Check Validation - You must inactivate a subject who is no longer associated with your operation within 7 days of leaving

There are multple caregivers who are no longer employed at the center whose background checks have not been inactivated.

746.1315(b) - Each Caregiver and the Director Must Have Current Training in pediatric CPR

Two caregivers present at the center did not have a current CPR/First aid training.

745.641 - AP Background check results - Must receive notification prior to allowing subject to be present at your operation

A caregiver present with the children in the classroom does not have a cleared and active background check. The caregivers background check is still in pending status.

746.401(9) - Posting Requirements - List entitled "Current Employees"--sized at least 81/2 X 11 inches, printed legibly, include employee first and last name

The employee list posted at the center is not current. It contained staff members that no longer work at the center and did not contain multiple staff members who currently work at the center.

746.909 - Access to Personnel Records

The director was not present during the inspection and the staff member in charge was not able to access the directors file.

746.3415(7) - Employee Handwashing After Handling or Cleaning Bodily Fluids

One staff member did not wash their hands after wiping a child's nose.

746.1205(a)(2) - Responsibilities of Caregivers - Know number of children responsible for

During the walkthrough in three clasrooms the caregivers did not know the number of children they were resposbilbe for.

746.4901(a) - Active Play Surfacing In Use Zones - Loose Fill or Unitary Surfacing

It was found that one play structure with a slide outside does not have surfacing around or underneath the structure.

746.521(11) - Parent right to be free from retaliatory action

It was found that the operation violated parent rights when a child was terminated as a form of retaliation.

746.2419(5) - Requirements for Feeding Infants - Fed in Safe Manner

It was found that infants are laid flat in their crib and given their bottle to drink without any support.

746.2415(b) - AP Cribs - Only Snug Fitting Sheet and Crib Mattress Cover Allowed if Infant Younger than 12 Months of Age

It was found that an infant was placed asleep inside the crib with the bib on.
