746.909 - Access to Personnel Records

During the inspection the keys to cabinet/file were not available during the inspection preventing access to the personnel record.

746.2407 - Infant Care Area Furnishings and Equipment -Use of Safety Straps

I observed six children seated in a highchair table with safety belts and none of the safety belts were fastened. Two seat were missing safety belts and one seats safety belt was missing the clasp. This was corrected when licensing staff asked for the children to be removed from the seats and children were seated at an age appropriate table and chair set.

746.3417(3) - Children Handwashing After Toileting or Diapering

I observed a child's diaper being changed and the caregiver released the child to the classroom without the child washing their hands. This was corrected when licensing staff asked the caregiver to wash the child's hands.

746.307(c) - Parental Notification of Less Serious Injuries

It was found that a parent was not notified of their child's possible less serious injury.

746.3417(3) - Children Handwashing After Toileting or Diapering

During the walk through I observed diaper changing procedures and the caregiver did not wash the child hands after changing them. This was corrected when CCR staff informed caregiver that the child's hand must be washed after each diaper change.

746.5101(a) - Annual Fire Inspection - Before Initial Permit Issued and Every 12 Months

During the inspection it was found that the operation had not had a fire inspection since 02/22/2024, being past due for their annual inspection by 6 weeks.

746.1601 - Child/ Caregiver Ratio - 13 or More Children

During the investigation inspection it was observed that one of the classrooms was out of ratio with one caregiver outside with 23 children. This caregiver did have a co-teacher, however the co-teacher was inside of the classroom and not outside with the class. This class was over ratio by 5 children in a ratio that should have been 1/18, with a specficied age of 4 years old.

746.605(13) - Required Admission Information - School Information for School-age Child

While reviewing children's records there was a school age child without the school's name and phone number documented.

746.605(6) - Required Admission Information - Emergency Contact

While reviewing children's records there were two out of ten files without an address for the emergency contact.

746.605(1) - Required Admission Information - Name and Birthdate

While reviewing records one child's file out of ten did not have the child's name or date of birth on the enrollment form.

746.4501(b) - Chairs - Safety Straps Used if Required by Manufacturer

A table in use does not have straps that belong on the apparatus.

746.2426(a)(1) - Infant May Not Sleep in a Restrictive Device

Child Care Regulation came to the center at 2:53 PM. When CCR was completing a walk through a infant was sleeping in a swing. CCR asked how long the infant had been sleeping in swing and staff stated since 1:17 PM.
