During an investigation operation was found deficient when a caregiver did not use good judgment when allowing a pipe cleaner to remain tightened around child's neck for an extended period of time resulting in red mark.

A child was not supervised with physical proximity, auditory, or visual awareness when allowed to retreve a backpack from a classroom on the second story without a caregiver.

At time of inspection, a caregiver was unable to account for how many children she was responsible for. Caregiver reported 21 children when there was actually 17.
